- Diabetes Hope Scholarship Program provides financial assistance and resources to youth with type 1 diabetes who exemplify leadership, giving them the confidence to successfully transition to post-secondary school and adult diabetes care.
- Hope Connects Peer Support Program ensures youth living with type 1 diabetes can connect with those who ‘get it’, through one-to-one peer mentor matches, monthly drop-ins, free online resources and our new online support forum.
- DHF Transition Resources: DHF has worked with our youth leaders and clinic partners to continue developing essential transition webinars and downloadable resources, all focused on ensuring students and their supporters are empowered with the information they need to help make their transitions just a little bit easier.
- DHF Youth Events: Free in-person events give youth with diabetes an opportunity to connect with one another and build their local community of support. Events include creative arts workshops, summer youth days and youth-led symposiums.
