Mentorship Program Application

With a focus on supporting the mental health and well-being of students, the DHF Scholarship Program launched in 2011 as a volunteer-driven peer mentorship program. The program was designed to provide academic and social support during the transition to both post-secondary education and adult diabetes care. Our peer mentors provide a safety net to new students, offering advice and guidance based on shared lived experiences to help students move towards independent diabetes management. Apply for a mentor using the HOPE Connects Mentee Application form below!
