Guide for Successful Transition to College & University

In the fall of 2022, we celebrated Diabetes Awareness Month with the launch of our online Transition Guide for students and young adults living with diabetes! The guide was developed by youth with T1D for secondary students preparing to transition to adult diabetes care and post-secondary studies. Designed for busy students on the go, DHF’s Transition Guide features:

  • A searchable database on the home page to easily find the school you’re interested in
  • Information about the clsoest diabetes education centre, endocrinologist and hospital
  • Campus supports for students with diabetes, including mental health services and diabetes clubs
  • Accessibility benefits available and how to access them
  • Quick online links to access mental health support services, health and dental plans available at their choice of college or university
  • Exam accommodations available through accessibility services
  • Alumni insights from students living with diabetes across Canada.

Our goal is to help youth with T1D make more informed choices about where to go next year based on the resources available at college and university campuses across the country.
